Abstract : To report a 65-year-old male who presented with subacute encephalopathic syndrome without preceding triggering factors. The diagnosis of an acute disseminated encephalomyelitis was based on suggestive MRI features, and confirmed by histopathological examination. Although of a rare occurrence, and such atypical presentation in this age group, this entity should be considered in the differential diagnosis of encephalopathy in elderly.
